License Types
There are four license types available for physicians in North Dakota:
Current, Resident, Locum Tenens, and Permanent.
Application Form
If you're hip you can fill out the online form, or you can fill out the paper version.
Reactivation
It's okay. People make mistakes. Maybe your license got inactivated. Or you left the state for a while. We don't ask questions. Actually, we do ask questions.
